Canadian Passports

1- Completed and Signed Passport Application Form

2 - Submit Your Canadian Passport
  Provide any previous Canadian travel document issued to you within the last five years such as a passport, a certificate of identity or a travel document.

3 - Completed and Signed Authorization Letter

4 - Most Recently Issued Passport
  Passport was expired less than five years ago and the bearer was over the age of 16 when the current passport was issued.

5 - Proof of Canadian Citizenship
  For persons born in Canada, you must provide either your:

• Birth certificate (issued by the province or territory of birth) or
• Certificate of Canadian Citizenship

Since November 26, 2001, only the following identity documents are accepted as proof of Canadian citizenship for Quebec-born citizens.

• A birth certificate issued after January 1, 1994 by the Directeur de l'etat civil in the province of Quebec.
• A Certificate of Canadian Citizenship.


Birth certificate for children under 16 years of age:

A long form birth certificate must be presented in the case of a child's passport application where the parents are not married or were not married at the time of the child's birth. This requirement allows Passport Canada to confirm the identity of both parents.

For persons born outside Canada, you must provide one of the four citizenship documents below:

• Certificate of Canadian Citizenship
• Certificate of Naturalization
• Certificate of Retention of Canadian Citizenship
• Certificate of Registration of Birth Abroad

6 - The Application Form and the Back of One of the Photographs Must be Signed by a Guarantor
  A guarantor is a person who can confirm your identity or that of your child. Your guarantor must perform the following tasks:

• Certify the information on the application form by completing and signing the "Declaration of Guarantor" section.
• Write on the back of one of your or your child's photos, "I certify this to be a true likeness of (your or your child's name)" and sign.
• If applicable, sign and date the photocopy of your supplementary documentation.

Eligible guarantors for Canadians residing in the United States
Your guarantor must:

• Be a citizen of Canada or the United States and must be accessible to Canadian Consulate for verification.
• Have known you personally for at least two years.
• Must be included in one of the following groups:

Chiropractor, Dentist, Judge, Lawyer, Magistrate, Medical doctor, Notary public, Pharmacist, Police officer, Veterinarian, or Signing officer of a bank or trust company, or of a financial institution that offers the full range of banking services (cash withdrawals, deposits and savings)

7 - Three (3) Identical Passport Photographs
  Acceptable Passport Photographs:

• Photographs must be taken within the last 6 months. If applying for a replacement passport for a child, you must include a photo that has been taken within the last month.
• Photos must be taken by a commercial photographer.
• The name of the photographer or studio, the address and the date the photo was taken (not the date the photo was printed) must be provided directly on the back of one photo (see illustration). Stick-on labels are unacceptable. Sufficient space must be allowed for the name of the applicant, the signature of a guarantor and the guarantor's declaration.
• Photos must be originals, not taken from any existing photo.
• One photo must be signed by a guarantor as a true likeness of the applicant. The guarantor should state the following:

I certify this to be a true likeness of

____________________ (name of applicant)

___________________ (guarantor's signature.)

• Notarial seals must not be imprinted on photos.

• Eyes must be open and clearly visible.
• Glasses, including tinted ones with prescription, may be worn as long as the eyes are clearly visible. Sunglasses are unacceptable.
• Facial expression must be neutral (not frowning nor smiling) with the mouth closed.
• Photos must show both edges of the face clearly.
• Photos must show a full front view of face and shoulders squared to the camera.
• The face and shoulder image must be centered in the photo. " Photos must reflect/represent natural skin tones.
• The length from chin to crown of head (natural top of head) must be 31 mm to 36 mm.
• The image must be clear, sharp and in focus.
• Photos with shadows on the face or background are unacceptable.
• Photos must be taken with uniform lighting and not show shadows or flash reflection on the face and head.
• Photos must be taken against a plain, uniform, white or light-coloured background.
• Hats or head coverings are not permitted except when worn for religious reasons and only if the full facial features are clearly visible.
• Only the head and shoulders of young children must appear on the photo (e.g. no hands holding the child).

8 - A Photocopy of Front and Back of your Alien Registration Card (Green Card)
9 - Proof of Departure
  Please provide a copy of your airline ticket or travel itinerary.

Notes:
  1. Place of birth:
Your place of birth must be provided on the application form.

2. For United States residents:
If you were accepted into the United States as a permanent resident before February 15, 1977, you must indicate on your application form the number and date of issue of your Alien Registration Card (Green Card). Include a copy of this card with your application form.

3. To use your married name
If you want the passport in your married name, you must complete the requested information about your marriage. Please provide Original or Certified Copy of your Marriage Certificate.

4. Sign in the white box
Your signature must not touch the border

5. References
Your guarantor should not be used as a reference. Relatives should not be used as references. A friend or co-worker is best.

6. Children's application
If applying for a child under 16 years of age, you must use the child passport application form and ensure that all legal documents regarding custody are submitted.
